An employee was replacing a windshield wiper arm on an up-armored Humvee. The employee pulled on the wiper blade arm and it broke free, causing the employee to lose balance and fall 3 feet to the maintenance bay floor. He sustained a fractured right shoulder and bleeding in the head.

An employee was cutting concrete form stakes out of 2"x6" lumber laying on a couple of sawhorses. There was a nail embedded in a 2"x6" that caused the saw to bounce when he cut into it. The saw bounced down onto his right leg, cutting his right inner thigh and requiring surgery.

He turned to speak to another employee who had spoken to him; while doing so, he bumped his hard hat, causing him to slip and fall to the ground. He fractured his right arm and left elbow. He was hospitalized.
